The platinum's real difficulty is concentrated in one trophy: Foehammer, the Gold reward for completing the entire game on Honour Mode. Honour Mode is Larian's hardest setting — a single shared save with no reloading and party-wide permadeath, where a full party wipe ends the run and the save converts out of Honour Mode, forcing a fresh attempt. That single requirement is why the platinum is rated so highly; the other 54 trophies are not hard to earn individually but demand knowledge and preparation. Note the important distinction on save-scumming: on normal, Balanced, or Tactician difficulty you CAN freely reload to fish for skill checks, dialogue outcomes, and choice-based trophies, but in Honour Mode you cannot — that run has to be played straight (some players back up cloud saves as a grey-area safety net, but the intended experience is one continuous save).
Because the game is act-structured with no way to revisit earlier acts and no post-ending open world, a large share of trophies are effectively missable and mutually exclusive across a single run — you generally cannot see both sides of a major story fork, romance every companion, or satisfy every 'save this character' condition on one file. This is why most trophy hunters plan for multiple playthroughs rather than one: a common approach is a relaxed first run at your own pace, then one or more focused runs on the easiest (Explorer) difficulty with a checklist open to mop up the branch-specific and choice-dependent trophies quickly, saving the demanding Honour Mode run for when you know the game's fights well.
An honest recommended path: play your first run for enjoyment on a comfortable difficulty and grab whatever comes naturally while save-scumming choices; do a second (and if needed third) efficient cleanup run on Explorer, flipping story decisions and romances against a trophy list to catch act-gated items; and leave Honour Mode for last, only attempting it after you understand encounter mechanics, since rushing it is the fastest way to lose the run in Act 1.
